Requesting a Vote-by-Mail Ballot

A Vote-by-Mail ballot may be requested for specific elections, or for all elections through the end of the calendar year of the next scheduled general election. A Vote-by-Mail ballot request for a voter may only be made by the voter, the voter's legal guardian, an immediate family member of the voter or the voter's spouse, or by a designee for a voter with a disability.  

"Immediate family" is defined as:  

  • Child
  • Grandchild
  • Grandparent
  • Parent
  • Sibling
  • Spouse
  • Spouse's grandparent
  • Spouse's parent
  • Spouse's child
  • Spouse's sibling

A request from the voter must include their:

  • Name
  • Address
  • Date of birth
  • Signature (written request only)
  • Florida driver's license, Florida ID card number, or the last four digits of their Social Security — Number whichever may be verified in the voter's record

Per Florida Statute 101.62, all written requests must be made using Form DS-DE 160 (Statewide Vote-by-Mail Ballot Request Form). This includes requests by fax or email. 

If a voter requests that a ballot be mailed to an address not on file in the voter's Registration record, the request must be made in writing using the Statewide Vote-by-Mail Request Form and signed by the voter.

A person may make a Vote-by-Mail ballot request for an immediate family member in person or by phone to an address already on file in the voter's Registration record without a signature. The person making the request must provide their:

  • Name
  • Address
  • Relationship to the voter
  • Driver's license number, or ID card number, or the last four of their social security number (if available)


 Along with the voter's:

  • Name
  • Date of birth
  • Address
  • Florida driver's license number, or Florida ID card number, or the last four digits of the voter's social security number, whichever may be verified in the voter's record
